This page contains all of the forms required pertaining to any breach of our Code of Behaviour or regarding match, venue and player movement requests.

It is important that clubs are fully aware of the Club and Player Guidelines which outline the requirements for each issue before they submit a request.

Forms for Clubs

Player Grading and Dispensation Requests

Please use the old form of the link fails on buttons opposite. This is a temporary issue.

  • Request a player re-grade or double-up for a weekend.
  • Regrade request for an Open Representative player due to lack of form or returning from prolonged injury.
  • Request an ineligible player to play in a final.
  • Dispensation for a junior player who is too young to play in a senior grade.

Change of Match Request

  • Change the ground at the same venue scheduled. I.e., moving a match from Griffiths Park 1 to Griffiths Park 2 – only available at venues which have multiple fields available.
  • Change times or ground allocation other than what is scheduled. This must be made at least 2 weeks in advance of the scheduled match.

Weather affected Match Information

Report on the condition of the grounds due to a weather event. This form lets The Fixtures and Grounds Sub-Committee know if a ground may be washed out or as a heads up for possible cancellation of a match. Please report no later than the Thursday evening prior to the weekend. Multiple reports may be made as conditions change.

Change of Fixture Request

  • Request a change of fixture (start time, change of location etc.). This request must be made at least 2 weeks prior to the scheduled match.

Impending Forfeiture

  • Inform of an intended forfeit of a match. This must be sent in no later than the Thursday midnight prior to the match.

Report and Response Forms

Incident Report Form – To be completed by anyone (including umpires) who wished to lodge a report they feel breaches the CFN code of conduct.

Charge Response – Any person/player who is charged must completed the form 1 within 48 hours of receiving charge. If the person being charged pleads not guilty, they must also complete form 2 within 48 hours of completing form 1. Anyone wishing to provide evidence in regards to an incident, please also complete form 2 within the required timelines.

Charge Findings – This form is to only be completed by the match review committee and/or commissioner/s as per the Code of Conduct.
