Club Appeal on an Incident Report Finding

This form is to be completed by the Club in response to and a disagreement on a finding made by the Conducts Panel or the Commissioner and would like to lodge an appeal to the finding report.

Once an appeal is made, the Operations Manager or Secretary will inform the club of the date and time of the appeal hearing.

In order for the appeal to go ahead, the club will be invoiced for $300.00 which must be paid before the appeal can go ahead

This form must be completed and returned within 48 hours (no later than Thursday midday following the incident) after the receipt of the Incident Findings Report.

Acknowledgement

As outlined in the 2026-2027 Code of Conduct approved by the board at their meeting on September 16, 2026.
The grounds for appeal are:
<ul>
 	<li>a material procedural error,</li>
 	<li>an error affecting the finding or grading,</li>
 	<li>a clearly inappropriate sanction, or</li>
 	<li>significant new evidence not reasonably obtainable earlier.</li>
</ul>
Mere disagreement is insufficient.

The appeal fee is $300 per appeal notice covering one person’s determination, not per individual ground or breach. Pay with lodgement, or provide payment evidence. A club may pay on the person’s behalf; a person without a club may pay directly. Administration issues payment instructions promptly. Where CFN’s invoicing or a verified banking problem prevents payment at lodgement, the Appeal Panel Chair may set a short written extension, ordinarily before the hearing.

Without payment or an approved extension, the appeal will not proceed.

Refund the full $300 where the appeal succeeds wholly or materially in part, including a finding set aside, grading reduced, sanction reduced or a rehearing ordered for material error.
