Club Response to Incident Form

This form is to be completed by the Club in response to the receipt of an Incident Report Form.
Clubs are asked to discuss the incident with their member and inform the CFN Match Review Committee of their response to the charges.
CFN requires a separate response to each individual reported.
This form must be completed and returned by midday of the Wednesday in the week following the match.

The alleged transgressor’s charge level:
At least one option must be chosen. If the person in question has received a Section 1 and Section 2 charge, choose the Section 2 option.
Please ensure you are very clear in the reasons for challenging the report.

Maximum of 2 files each being no larger than 2Mb.

A maximum of 4 documents not more than 2Mb in size may be uploaded.
